Washington’s My Health My Data Act and Nevada’s consumer health data law give you specific rights over your consumer health data. Below I explain the consumer health data Moss Herbalism collects, how I use and share it, and how you can exercise your rights. My Privacy Policy covers everything else.
What I collect and why
Consumer health data is personal information that identifies your past, present, or future physical or mental health. I collect these categories:
| Category | What it includes | How I use it |
|---|---|---|
| Health information from your intake questionnaire and consults | Age, health history, symptoms and conditions, past diagnoses, medications and supplements, allergies, pregnancy, breastfeeding, and menstrual health, mental and emotional health, sleep, digestion, nutrition, stress, movement, the other practitioners you see, and your goals | To prepare for and hold your consults, write your wellness plan and formula, check for herb and medication interactions, and follow up with you |
| Booking details | Your name, email address, appointment type and time, and answers to booking questions | To schedule, confirm, and hold your consult |
| Purchases | Products, formulas, courses, and memberships you buy, with your shipping details | To fill, ship, and support your orders |
| Site and email activity | Visits to pages about specific health topics, with your IP address, device details, and cookie identifiers; opens and clicks in my emails | To understand which topics help people, and to improve my site and emails |
Where it comes from
Directly from you, when you book, answer booking questions, fill out my intake questionnaires, meet with me, email me, or place an order. From the services that run my site, booking, and shop, such as Calendly, Google Analytics, and Mailchimp, when you use my site or open my emails. And from your browser and device when you visit my site.
What I share and with whom
I share consumer health data only as needed to provide the services you ask for, or with your consent. I never sell it.
| Data shared | Who receives it | Why |
|---|---|---|
| Booking details and answers to booking questions | Scheduling service (Calendly) | Booking and reminders |
| Your consult conversation | Video conferencing service (Zoom) | Holding virtual consults |
| Intake questionnaire answers and health details you email me | Form tool (WPForms), email delivery service, and email provider (Google) | Getting your answers and messages to me |
| Purchase and shipping details | Payment processor (Stripe), shop and shipping services (WooCommerce), and shipping carriers | Payment and delivery |

Google Analytics and Mailchimp’s site code can recognize your browser across other websites over time. Moss Herbalism has no affiliates. I also disclose consumer health data when the law requires it.
Your rights
If you’re a Washington or Nevada consumer, you have the right to:
- confirm whether I collect, share, or sell your consumer health data, and get a copy of it
- receive a list of every third party I’ve shared it with, and how to contact them
- ask me to correct it
- withdraw your consent
- ask me to stop collecting or sharing it
- have it deleted, including by the services that hold it for me
Using these rights is free and never changes how I treat you.
